Vestibular therapy, explained

Vestibular therapy treats the cause of dizziness, not just the symptom.

Vestibular rehabilitation therapy is a specialized form of physical therapy that retrains the body’s balance system to reduce dizziness, vertigo, and unsteadiness. Through targeted eye, head, posture, and movement exercises matched to the cause of your symptoms, it helps the brain adapt to, compensate for, and habituate the faulty signals that keep you off balance.

Dizziness has many sources: the inner ear, the visual system, the neck, the brain’s processing of motion, or a combination of these. That is why Dr. Crespo begins every case by recording what your eyes do under infrared lenses. The eye-movement pattern helps indicate the cause of dizziness.

The Plan

A three-step diagnostic process

Before any treatment, Dr. Crespo follows the same three steps to locate the source of your dizziness

01

Infrared Video Frenzel Assessment with dynamic Functional Equilibrium Assessment

Infrared lenses remove visual fixation so involuntary eye movements that are invisible in normal light become recordable. Findings are paired with dynamic balance and equilibrium testing.

  • Spontaneous and gaze-evoked nystagmus
  • Positional testing for canal involvement
  • Functional equilibrium and VOR behavior
02

Analysis of Nystagmus patterns

Peripheral vestibular dysfunction, central vestibular mechanisms, or cervical proprioceptive contribution.

  • Peripheral: organ involvement vs. hypofunction / neuropathy
  • Central: post-concussion or central neurologic mechanisms
  • Cervical proprioceptive deficits
03

Cervical Orthopedic Examination: Craniofacial / TMJ / Cervical Spine

In this step Dr. Crespo connects vestibular findings with cervical mechanics, craniofacial pain, and TMJ behavior, then sets the direction for treatment.

  • Craniofacial and TMJ evaluation
  • Cervical spine mechanics
  • Targeted manual and rehabilitative care

Common questions about vertigo, dizziness, and vestibular therapy.

What is vestibular rehabilitation therapy?

Vestibular rehabilitation therapy is a specialized form of physical therapy that retrains the balance system through eye, head, posture, and movement exercises matched to the cause of dizziness. It works by helping the brain adapt to, compensate for, and habituate the faulty signals that cause vertigo and imbalance.

How do I know if my dizziness is BPPV, vestibular migraine, or PPPD?

The pattern is the clue. BPPV produces brief, intense vertigo triggered by specific head positions and is the most common cause of vertigo. Vestibular migraine brings dizziness and motion sensitivity, with or without headache, often alongside light and sound sensitivity. PPPD feels like persistent rocking, swaying, or unsteadiness that lasts most days and worsens with upright posture, motion, and busy visual environments. Dr. Crespo’s assessment identifies which mechanism is dominant rather than guessing from symptoms alone.

What is BPPV, and can it be fixed?

BPPV (benign paroxysmal positional vertigo) is the most common cause of vertigo. It happens when tiny calcium crystals shift into a semicircular canal of the inner ear and send false motion signals. It is treated with canalith-repositioning maneuvers such as the Epley maneuver, which guide the crystals back where they belong. In experienced hands these maneuvers resolve most cases, often quickly.

Why use infrared Video Frenzel testing?

Infrared recording removes visual fixation, which normally suppresses abnormal eye movements. In darkness, nystagmus that is invisible in a lit room becomes recordable, and those patterns help localize whether the problem is peripheral vestibular, central, cervical, or mixed. Research suggests roughly two-thirds of abnormal eye movements are missed without infrared goggles.

When should I book an assessment?

Book if dizziness keeps returning, if home maneuvers did not resolve your symptoms, if you have imbalance after a concussion, or if your vertigo comes with migraine, neck, jaw, or facial pain. An assessment gives you an answer about the cause before committing to a treatment plan.
